KEYNOTE SPEAKERS
Don Bradley is an Associate Consulting Professor in the Department of Community and Family Medicine at Duke and the Director for the Practical Playbook (www.practicalplaybook.org). He retired from Blue Cross and Blue Shield of North Carolina (BCBSNC) in 2014, where he served in a number of roles, including Executive Director for BCBSNC’s federally qualified Health Maintenance Organization, and as Senior Vice President, Healthcare and Chief Medical Officer.

José A. Pagán, PhD, is Director of the Center for Health Innovation at The New York Academy of Medicine. Before joining NYAM Dr. Pagán was the Principal Investigator of a three-year, $7.3 million Health Care Innovation Award from the Centers for Medicare & Medicaid Services to develop and implement the Brookdale Senior Living Transitions of Care Program. He was Professor and Chair of the Department of Health Management and Policy, School of Public Health, University of North Texas Health Science Center.
