Funding provided by the Office of Community Health and Performance Management in the Nebraska Department of Health and Human Services and the Office of Public Health Practice, College of Public Health at the University of Nebraska Medical Center.

Purpose: To share best practices, lessons learned, and critical success factors in developing the CHNA and implementing action plans to meet the high priority needs of the population. It will also explore how these activities can become more effective in improving population health outcomes.

Who should attend: Hospital representatives who are involved the CHNA and implementation activities; local and state health department staff who support hospitals in these efforts; other local and state partners who are involved or support these planning and implementation efforts.
